Member Recognition Form

You are invited to suggest recognition for current members of NETC. Recognitions may be submitted throughout the year and will be shared at NETC Monthly Gatherings and during the annual NETC conference. Recognitions may include (but not limited to) milestones such as:

  • Promotions
  • Milestone work anniversaries
  • Special awards received
  • Retirements
  • Passing/Remembrances

All form fields are required for submission. Every submission must include:

  • Name of person being recognized
  • Work title
  • Name of employing institution
  • Reason for recognition
  • Specific words to use during the public recognition
  • Name of person submitting the recognition
  • Email of person submitting the recognition
  • Consent by the nominee to post on the NETC website, Facebook, and Twitter accounts, and to share at a NETC monthly gathering and the annual conference.
  • By submitting this form, I am verifying that the information is true and correct.

The NETC Board reserves the right to make final decisions.
